- ONLINE INFO CHANGE FORM (to be completed by Benefit Rep / Employer)
- LIFE INSURANCE COVERAGE WHEN LEAVING EMPLOYMENT (optional – you can convert your group life insurance to an individual plan upon termination of employment for employees below age 67)
REMINDER: If you have enrolled in Voluntary Critical Illness, please contact Industrial Alliance to cancel the coverage and premium deduction.
