- You must be Actively at Work

| Laser eye surgery, eyeglasses and contact lenses | 80% of the covered costs up to $300
$25 annual extended health deducible |
| Eye examinations | 100% of the covered costs up to $65 in any two consecutive calendar year period for age 21 and over, and every calendar year for under age 21.
